Meet Jean Zaleski

Jean Zaleski, PT, MEd, DPT is a physical therapist with more than two decades of experience in home health as clinician and clinical leader. Her career focus includes provision of high-quality patient centered care and documentation, promotion of cross continuum care transitions, chronic disease management and prevention of injury and rehospitalization.

Jean has served in leadership positions at a number of New England based home care agencies. These include Process Improvement and Project Development Manager with Holyoke VNA and Hospice Life Care, Director of Clinical Practice at Mercy Home Care, and Director of Performance Improvement and Documentation at VNA Care Home Health and Hospice.

Her core areas of expertise include quality and process improvement, highly developed assessment and auditing skills, regulatory adherence and an evidence-based approach for education.

Jean is a graduate of Quinnipiac University with a Batchelor of Science degree in Physical Therapy.  She earned a Master’s degree in Rehabilitation Services from Springfield College and a Doctor of Physical Therapy at the Massachusetts General Hospital Institute of Health Professions. She is a resident of Westfield, Mass.
